In Switzerland, the number of people with chronic illnesses is steadily increasing. The Chronic Care Management model opens doors to support general practitioners with specially trained practice assistants and to provide targeted, individualised care to patients in managing their chronic illnesses.

Our interdisciplinary team provides high-quality treatment, ensuring competent outpatient primary care for patients. The aim is to promote self-management and self-competence among patients, enabling them to keep their condition under control and recognise warning signs. Patients receive long-term support and we offer targeted treatments and therapies. When it comes to your health, we want to be your first point of contact. You will receive personalised advice from doctors and trained medical practice assistants on how to deal with the consequences of your illness and on preventive measures.

We offer support in the following areas:

  • Type 2 diabetes mellitus
  • Bronchial asthma (regular lung function checks, instruction and support in the use of various inhalers, information in acute cases, preventive measures)
  • COPD (regular lung function checks, instruction and support in the use of various inhalers, preventive measures, oxygen saturation monitoring)
  • Nutritional counselling/psychology
